La familia Hess, julio 2026

Our Healthcare District board should direct MarinHealth Hospital, not take its direction. A public board answering to executives isn’t a watchdog, it’s a lapdog. I bring independent oversight and audit.

First, transparency: put every major capital ask and tax proposal under public light — who drafted it, who funds it, and what the last levy delivered — before residents are asked to pay more.

Second, accountability: 92% of U.S. hospitals thrive without taxing power. With $24M surpluses and $2M+ executive pay, unnecessary taxes raise costs and rents for working families, taking more from those with less. For these, public health doctrine preaches the time-tested truth that wealth brings health: tax one, and you tax the other.

Third, checks and balances: The hospital CEO currently acts as executive for the public board elected to oversee him. We need true division of power.

I am a board-certified radiation oncologist with a Harvard master’s in public health, unbought and independent. Enterprise unlocks the California Dream for everyone healthy enough for the pursuit. When the body breaks, physicians restore that personal health. When our system breaks and takes from those it was meant to serve, say “Yes” to independent oversight and restoring public health.

Dr. Clay Hess, MD, MPH

MarinHealth: excellent care but not fully honest about the bill.

I wish I didn't have to write this...

Quisiera no tener que escribir esto...

Other hospitals deliver excellent care without any of this:

  1. A $2.1 million CEO. MarinHealth paid its CEO $2.1 million in 2024, according to its IRS Form 990.1 That's above the $2.0 million line Cause IQ reports for the top 5% of U.S. nonprofit hospital CEOs.2

UU.2

  3. An $8 million strikeout. After a one-day strike in February, MarinHealth locked out about 1,200 nurses and staff, members of the California Nurses Association and Teamsters Local 856, for two more days.3 MarinHealth told the district board the three-day strike and lockout cost about $5 million in replacement labor, an $8 million hit in all.4 That's nearly a third of the hospital's entire 2025 net income, gone in three days.5

  5. A hospital tax billed as a “citizens’ initiative.” A MarinHealth Foundation board member put up $573,000 of the $737,500 raised for Measure CC, and the committee paid a Las Vegas firm $244,502 to collect signatures.6 At the district's Feb. 24, 2026 board meeting, director Dr. Edward Alfrey said he overheard petition gatherers telling people that if they didn't sign, “the hospital will go under,” and called that “straight up dishonest.”7

  7. A rebuild pitch without the analysis. The ER and trauma center are already in the 2020 Oak Pavilion, which has the state's top structural seismic rating.8 The building facing the 2030 seismic deadline is the Redwood Pavilion, built in 1952, and it doesn't house the ER or the trauma center.9 MarinHealth says Redwood can't reasonably be retrofitted and should be replaced, at $350 million to $550 million, but it hasn't published an analysis comparing retrofitting Redwood with replacing it.10 And Measure CC doesn't require a dollar to go to seismic work.11

I'm an independent physician, and I don't practice at MarinHealth.13 After fifteen years at the bedside, I know that excellent care and honest care go together. Patients deserve both, and so do the taxpayers who own this hospital.

Our Healthcare District board should direct MarinHealth, not take its direction. Today MarinHealth's CEO is also the district's CEO, serving as executive for the public board elected to oversee him.14 A public board answering to executives isn't a watchdog. It's a lapdog.

What Measure CC asks

Lo que pide la Medida CC

A blank check — broad authority without a narrow, itemized ask for what safety actually requires.

What I favor instead

Lo que yo favorezco

A narrow check — targeted funding for the earthquake retrofits that are needed, and only that which is absolutely needed.

The hospital should be asking for what it needs, not what it wants. It admits that it does not need the tax; it just wants a cushion.

Fiscal responsibility means asking voters for only that which is absolutely needed — not a blank check dressed as earthquake safety. Needed retrofits deserve a clear, narrow ask. Measure CC is not that ask.

The vacant Division 1 seat

El asiento vacante de la División 1

When Director Ramirez resigned in July, the board could have appointed someone to Division 1 until December. On July 14 the board discussed it without a vote. Some directors preferred to let the November election decide; one said he would “much prefer appointing someone than having the county appoint somebody.” A director also said “it’s been hard to get really good people to run for the district board.” Later that meeting I put my name forward: I am a physician with an M.D. from Penn State and an M.P.H. from Harvard, and I live in Division 1. The next day I applied in writing, and the district confirmed it received my application. On Aug. 11, when I asked about the appointment, the chair said the “general consensus” was “that the election would be in November and we would let that decide that.” The July 14 minutes record no vote or decision, and the board has not taken up an appointment since. Division 1 has had no representative on the board since July.

The lease has handcuffed the public board. A dual CEO role is a conflict of interest.

“A public board answering to executives isn’t a watchdog, it’s a lapdog.” The hospital CEO currently acts as executive for the public board elected to oversee him.

That they are returning to ask the public for another 30-year tax is evidence capital funding is not going well — otherwise they would not be proposing a double tax (a new Measure CC parcel tax on top of existing hospital debt service and prior capital asks).

Fielded Questions

When anyone asks me a question, everyone hears the answer. Here are questions voters have sent me, and my replies.

Asked by: A MarinHealth nurse

  1. Question 1

If bedside clinicians bring a patient-safety concern to the District Board after it wasn't resolved internally, what is the board's responsibility to follow up?

    Dr. Hess answers

    1. Board follow-up. When nurses or other bedside clinicians bring a patient-safety concern to the District Board because they believe it wasn't resolved internally, the board has a duty to follow up. That means asking management for a written response, setting a date to report back in public, and checking that the issue was actually fixed. The board answers to the public and to patients, not to hospital management.

  2. Question 2

Would you support a process for frontline employees to raise serious patient-safety concerns with the District Board without fear of retaliation?

    Dr. Hess answers

    2. Raising concerns without retaliation. Yes. I would support a clear, protected way for frontline staff to bring serious patient-safety concerns to the board, including confidentially, with a firm commitment against retaliation.

  3. Question 3

If several nurses report concerns about staffing, training, changes in clinical practice or working conditions that could affect patient safety, would you support an independent review rather than relying only on administration's assessment?

    Dr. Hess answers

    3. Independent review. Yes. When several nurses report related concerns about staffing, training, changes in clinical practice or working conditions, the board should not rely only on administration's own assessment. An independent clinical review is the right tool, and it's part of the independent oversight and audit I've called for.

  4. Question 4

Would you support tracking recurring patient-safety and staffing concerns raised during public comment, with required follow-up?

    Dr. Hess answers

    4. Tracking public comment. Yes. Patient-safety and staffing concerns raised in public comment should be logged, assigned for follow-up, and reported back at a later meeting so they don't disappear when the comment period ends.

  5. Question 5

How would you keep the District Board independent when evaluating concerns involving MarinHealth's executive leadership?

    Dr. Hess answers

    5. Independence from executives. The board's job is to oversee leadership, not to defend it. I'm an independent physician and I don't practice at MarinHealth, so I don't owe my position to hospital management. On the board I'll ask hard questions in public, push to publish the information the community deserves, and bring in independent review when concerns involve executive leadership.

Preguntas Respondidas

They Refused to Read Your Letters in August · We Return in September

Se Negaron a Leer Sus Cartas en Agosto · Volvemos en Septiembre

FILL THE SEAT

Thank you to every neighbor who submitted a letter for the August 11 meeting. The board refused to read them or enter them into the public discussion — with the Division 1 seat still vacant. So we say it again in September: read the letters, fill the seat, appoint Dr. Hess.

When Director Ramirez resigned in July, the board could have appointed someone to Division 1 until December. On July 14 the board discussed it without a vote. Some directors preferred to let the November election decide; one said he would “much prefer appointing someone than having the county appoint somebody.” A director also said “it’s been hard to get really good people to run for the district board.” Later that meeting Dr. Hess put his name forward: he is a physician with an M.D. from Penn State and an M.P.H. from Harvard, and lives in Division 1. The next day he applied in writing, and the district confirmed it received his application. On Aug. 11, when Dr. Hess asked about the appointment, the chair said the “general consensus” was “that the election would be in November and we would let that decide that.” The July 14 minutes record no vote or decision, and the board has not taken up an appointment since. Division 1 has had no representative on the board since July. Neighbors are writing again: fill the seat on the merits.

A Plan for Every Neighborhood

Un Plan para Cada Barrio

Division 1 is not one place. Each neighborhood pays differently, needs differently, and wants to engage its healthcare differently. Here is how.

Help that arrives directly; not in thirty years.

What you pay

Lo que paga

Nearly half the Canal rents, so district costs often arrive as rent, because landlords pass them through. The Canal is also the most industrial and commercial corner of Division 1 (its single most common zoning is light-industrial/office), so those costs land on the warehouses and shops that employ people here, too.

What you want

Lo que quiere

Care you can reach this month; not a promise dated 2056. Rent at $2,350 for a one-bedroom does not wait for a bond cycle. And a real say in a district that reaches you through your landlord.

The plan

El plan

Two things, not one: a seat that answers to renters as well as owners; and kindMANkind, neighbor-to-neighbor help that arrives now rather than in 2056.

kindMANkind is a platform for peer-to-peer humanitarianism. Refillable, trackable debit cards let a sponsor send help directly to a specific neighbor and allocate it to a specific purpose, a prescription, a co-pay, a utility bill, with receipts verified, and the option to give remotely or anonymously.

It is a complement, never a substitute. Charity is not a healthcare system, and the Canal is owed a vote; not just a benefactor. But nobody in this neighborhood should have to wait thirty years for a hospital board to decide they matter.

Disclosure: kindMANkind is a venture founded by Dr. Hess. He receives no payment from its use.

Flat costs that ignore whether a shop had a good year still land on every storefront.

Downtown is the district’s biggest and most mixed-use bloc, from R5 lots to the Fourth Street commercial frontages. Flat per-building costs hit homes and storefronts — whether or not a shop had a good year.

What you want

Lo que quiere

A downtown where a small business can survive its first three years; and where the people who work here can afford to live here.

The plan

El plan

This is the mission at its sharpest: enterprise unlocks the California Dream. Defend the businesses that generate Marin’s health — do not grind them with flat costs that ignore whether they earned.

Costs charged on the space you occupy, not the success you had, are the opposite of enterprise; and downtown, with more parcels than any other neighborhood in Division 1, feels them first.

Mostly smaller R5 lots and older homes, with a real share of duplexes and small multifamily; so the bill lands on owners and renters alike, and on exactly the teachers, nurses, and firefighters with the least room left in the budget.

Everyday primary care you can reach without a referral maze; and a county your children can still afford to live in.

The plan

El plan

Push the district toward primary and preventive care close to home, the cheapest medicine there is, and treat every new levy as a cost of living, because it is.

Ask why your children and grandchildren cannot afford to live near you. Marin's middle class is missing, and raising taxes is the culprit; our path of least resistance. Gerstle Park is where that arithmetic bites first.

The hospital isn’t your neighbor. The bill still is.

El hospital no es su vecino. La cuenta sí lo es.

What you pay

Lo que paga

Dominican is zoned almost entirely for single-family homes, and on the larger lots in Division 1, R7.5 and R10 alongside R5. Because the levy is charged per square foot of building, the bigger the house, the bigger the bill, on top of the roughly $16 million a year Marin property owners already pay servicing the hospital’s bonds.

What you want

Lo que quiere

Your neighbor is a university, not a hospital, MarinHealth’s medical center sits in Greenbrae, outside San Rafael altogether. You are being asked to fund a campus you don’ t live beside, on a bill measured by your own roof. You want the evidence before the money.

The plan

El plan

Evidence first; the standard a university town should expect. Every capital ask arrives with an audited statement of what the last one delivered, and what the existing bond taxes already bought, before a single new dollar is levied.

Distance is the point. Capital asks that ignore how near you live to the emergency room, how often you use it, or whether you’re insured elsewhere still land hardest on Dominican rooftops — some of the largest bills in Division 1 for a medical center in another town.
